Product overview

A botulinum toxin type A supplied without complexing proteins

Xeomin is the botulinum toxin type A product manufactured by Merz Pharma in Germany. Its active substance is incobotulinumtoxinA, supplied as a sterile lyophilised powder in a single 100-unit vial for reconstitution with preservative-free 0.9 % sodium chloride before use.

What distinguishes Xeomin from most toxins in this category is its formulation. Manufacturing removes the accessory complexing proteins that surround the active neurotoxin in conventional preparations, leaving the neurotoxin itself with only human albumin and sucrose as excipients. Merz markets this as a purified formulation; the clinical significance of the difference remains a matter of ongoing discussion in the literature rather than settled fact.

A practical consequence of the formulation is storage. Unopened Xeomin vials do not require refrigeration in markets where the current label permits ambient storage, which simplifies stock handling and transport compared with toxins that must be held at 2–8 °C throughout. Always work from the storage section of the prescribing information approved where you practise.

Conventional excipient profile

Human albumin as stabiliser and sucrose as bulking agent — a short, well-characterised excipient list.

Reading the carton

Before you accept a delivery, check four things:

  • The lot / batch number on the carton matches the number printed on the item itself.
  • The expiry date leaves you enough working life — we do not dispatch stock with under 6 months remaining unless agreed in advance.
  • The seal is intact and the packaging has not been opened or disturbed.
  • The parcel arrived in the condition you expect for this product, including any coolant packs still being cold.

Need the batch details first? Ask our team before you order and we will confirm the lot number and expiry date of the stock allocated to you.

How to use

Preparation and administration

Reference information for licensed practitioners. Reconstitution and administration must follow the prescribing information approved in your market and your own clinical governance procedures.

Step by step

  1. Inspect the vial

Check the label, lot number and expiry date. The powder should be white and the vial undamaged.

  2. Draw the diluent

Draw the required volume of preservative-free 0.9 % sodium chloride injection into a sterile syringe.

  3. Disinfect the stopper

Wipe the exposed rubber stopper with an alcohol swab and allow it to dry.

  4. Introduce the diluent gently

Direct the saline against the vial wall rather than onto the powder. Discard the vial if there is no vacuum drawing the diluent in.

  5. Mix without shaking

Rotate the vial slowly until dissolved. The solution should be clear, colourless and free of particulates.

  6. Record and use promptly

Note the reconstitution date and time on the vial, keep it refrigerated at 2–8 °C, and use within the period stated in your approved prescribing information.

Dilution reference

Diluent added Units per 0.1 mL Total volume
1.0 mL 10.0 U 1.0 mL
2.0 mL 5.0 U 2.0 mL
2.5 mL 4.0 U 2.5 mL
4.0 mL 2.5 U 4.0 mL
5.0 mL 2.0 U 5.0 mL

Administration notes

  • Units of botulinum toxin are product-specific. Do not convert or substitute units between different botulinum toxin brands.
  • Dose per site, total dose and injection technique must follow the prescribing information approved in your market.
  • A fine-gauge needle appropriate to the treatment area is normally used, and reconstituted product is drawn immediately before injection.
  • Treatment intervals should not be shortened below the minimum stated in the prescribing information.
  • Discard the vial if the reconstituted solution is cloudy, discoloured or contains particles.
Storage & handling

Storage and handling

Unopened vial

Store in the original carton. Where the approved label for your market permits ambient storage, refrigeration of unopened vials is not required. Where it does not, store at 2–8 °C. Work from the storage section of the insert supplied with your stock — this is one of the few points where Xeomin differs from most toxins, and label wording has changed between revisions.

After reconstitution

Keep at 2–8 °C and use within the period specified in the approved prescribing information. Do not freeze reconstituted solution. Single-patient use only.

Disposal

Inactivate residual product and dispose of vials, needles and syringes as clinical waste in accordance with local regulations.

Cold chain integrity

Confirm the storage conditions on the carton you actually receive. Ambient storage is a property of specific approved labels, not of every Xeomin presentation in every market, and it is not an invitation to leave stock in a warm room indefinitely.

FAQ

Frequently asked questions

What is Xeomin 100U?

Xeomin 100U is a botulinum toxin type A product manufactured by Merz Pharma in Germany. The active substance is incobotulinumtoxinA, supplied as a sterile lyophilised powder in a single 100-unit vial and reconstituted with preservative-free 0.9 % sodium chloride before injection by a licensed practitioner.

What does "free from complexing proteins" actually mean?

In most botulinum toxin preparations the active neurotoxin is surrounded by accessory proteins from the bacterial culture. Xeomin’s manufacturing process removes them, leaving the neurotoxin with only human albumin and sucrose as excipients. Merz presents this as a purified formulation. Whether the difference changes clinical outcomes is still discussed in the literature rather than settled, so it is best described as a formulation characteristic rather than a proven advantage.

Does Xeomin need refrigeration?

This is where Xeomin differs from most toxins. Under the current labelling in several markets, unopened vials do not require refrigeration and may be stored at or below 25 °C. Older label revisions permitted room temperature, refrigerated or frozen storage. Because the wording has changed over time and varies by market, follow the storage section of the insert supplied with your stock rather than a general rule.

How is it reconstituted?

With preservative-free 0.9 % sodium chloride injection. Dilution volume is chosen by the practitioner: 2.5 mL into a 100-unit vial gives 4 units per 0.1 mL, which is a commonly used concentration. The reference table in the How to Use section lists other volumes. Always follow the reconstitution instructions in the prescribing information approved for your market.

Can I substitute Xeomin units for another brand's units?

No. Units of botulinum toxin are specific to each product and are not interchangeable between brands. Dosing must be taken from the prescribing information for the product in your hand.

Who is allowed to buy this product?

Botulinum toxin is a prescription-only medicine. Purchase and use are restricted to licensed healthcare professionals qualified to prescribe or administer injectable prescription medicines in their jurisdiction, with appropriate training in the relevant anatomy and injection technique. We do not sell to consumers.

Can I get the batch number and expiry before ordering?

Yes. Ask our team and we will confirm the lot number and expiry date of the stock allocated to your order before it is dispatched. We do not ship product with less than six months of remaining shelf life unless you have specifically agreed to it.
